What is an Abdomen and Pelvis Ultrasound-Scan?

The abdomen and pelvis ultrasound-scan is a comprehensive diagnostic procedure that evaluates the major organs in both the upper abdomen and the lower pelvic cavity during a single appointment.

When symptoms are vague, widespread, or overlapping — such as generalised abdominal pain, persistent bloating, or unexplained weight changes — it can be difficult to pinpoint the exact cause. A pelvic issue (like a large ovarian cyst) can cause upper abdominal discomfort, and an upper abdominal issue (like gallstones) can cause radiating pain.

By combining the abdominal scan and the pelvic scan, we provide a complete overview of your internal health, ensuring no potential causes are missed. The procedure uses high-frequency sound waves to create real-time, high-resolution images without the use of ionising radiation.

Common clinical goals

  • Investigate the source of widespread or radiating abdominal pain.
  • Determine the cause of persistent bloating or swelling.
  • Assess abnormal blood test results (e.g., elevated liver enzymes or inflammatory markers).
  • Rule out multiple conditions simultaneously for faster reassurance.

What the Scan Evaluates

Ultrasound scan of the upper abdominal organs
Upper Abdomen

Liver, Gallbladder & Pancreas

We assess the liver for size, texture, and signs of fatty infiltration. The gallbladder and bile ducts are checked for gallstones, sludge, or inflammation. The pancreas and spleen are evaluated for structural abnormalities or enlargement.

Ultrasound scan of the kidneys and urinary tract
Urinary Tract

Kidneys & Bladder

Both kidneys are measured and examined for cysts, kidney stones, or signs of obstruction (hydronephrosis). The bladder volume is assessed, and the bladder wall is checked for irregularities or thickening.

Transvaginal ultrasound scan of the pelvic organs
Reproductive

Uterus & Ovaries

The uterus is assessed for fibroids, adenomyosis, and endometrial thickness. Both ovaries are examined for cysts, follicle count (PCOS assessment), and structural normality. The pelvic cavity is checked for free fluid.

When should you book this combined scan?

This scan is highly recommended when symptoms do not clearly point to a single organ system, or when you want the peace of mind of a complete abdominal and pelvic review.

Common symptoms investigated:

  • Generalised abdominal pain or discomfort that moves or radiates.
  • Persistent bloating, swelling, or a feeling of fullness.
  • Unexplained weight loss or changes in bowel/urinary habits.
  • A combination of pelvic pain and upper digestive issues.

Important Preparation

Because this scan looks at both the upper abdomen and the pelvis, strict preparation is required to ensure clear images of all organs.

1
Fast for 4 hours Do not eat any food for 4 hours before your appointment. This ensures your gallbladder is full and your upper organs are not obscured by bowel gas. Clear fluids are allowed.
2
Drink 1 litre of water Drink 1 litre of water exactly 1 hour before your scan and do not empty your bladder. A full bladder acts as an acoustic window to view the pelvic organs clearly.

How Your Appointment Works

1
Arrival & History You will be welcomed at our Harley Street clinic. The sonographer will review your symptoms and medical history to tailor the scan to your needs.
2
Abdominal Scan While lying on the couch, warm gel is applied to your abdomen. The transducer is moved across your upper abdomen to assess the liver, gallbladder, pancreas, and kidneys.
3
Pelvic Scan The pelvic area is assessed transabdominally first. You will then be asked to empty your bladder, and with your consent, a transvaginal (internal) scan is performed for detailed views of the uterus and ovaries.
4
Immediate Feedback The consultant sonographer will explain the findings to you clearly during the appointment, ensuring you understand what has been seen.
5
Written Report A comprehensive medical report and digital images are provided on the same day, ready to be shared with your GP or referring specialist.

Frequently Asked Questions

Do I need a GP referral for an abdomen and pelvis scan?

No, you do not need a GP referral to book a private abdomen and pelvis ultrasound-scan at Sonoworld. You can self-refer and book your appointment directly online or by phone. If you do have a referral letter, please bring it with you to the appointment.

How should I prepare for the combined scan?

Because this scan assesses both the upper abdomen and the pelvis, you need to follow two preparation steps: fast for 4 hours before your appointment (to clear the gallbladder), and drink 1 litre of water 1 hour before (to fill the bladder for the pelvic assessment). Clear fluids like water or black tea are permitted during the fasting period.

Will I have a transvaginal (internal) scan?

Yes, for the most accurate assessment of the uterus and ovaries, a transvaginal (internal) scan is routinely performed as part of the pelvic assessment, following the transabdominal scan. This provides much higher resolution images of the reproductive organs. However, this is always done with your full consent, and you can decline it if you prefer.

Can I have the scan while on my period?

Yes, you can have the scan while on your period. It does not affect the quality of the images or the accuracy of the assessment. If you use tampons or menstrual cups, you will simply be asked to remove them before the transvaginal part of the scan.

How quickly will I get my results?

Your consultant sonographer will explain the findings to you during the appointment, so you will leave with a clear understanding of the results. A comprehensive written report and digital images will be provided on the same day, ready to share with your GP or specialist.
