The thyroid gland sits at the front of the neck and plays a central role in regulating metabolism, energy, and hormonal balance. When blood tests reveal abnormal thyroid hormone levels (TSH, T3, T4), or when a lump or swelling is felt in the neck, ultrasound is the first-line imaging investigation recommended by clinicians. It can characterise nodules, assess lymph nodes, and identify structural abnormalities with exceptional precision.
Unlike CT or MRI, thyroid ultrasound involves no radiation and no contrast agents, making it completely safe and suitable for all patients, including those who are pregnant.
We assess the size, shape, and echogenicity of both thyroid lobes and the isthmus. Any nodules are measured and characterised using the standardised TI-RADS classification system to determine their risk profile and whether further investigation is required.
The lymph nodes along the sides of the neck are systematically assessed for size, shape, and internal architecture. Enlarged or abnormal-looking nodes can indicate infection, inflammation, or, in some cases, more serious pathology requiring further investigation.
The parotid and submandibular salivary glands are evaluated for stones, cysts, or masses. Any palpable lumps or swellings in the neck soft tissue are also assessed, providing a comprehensive overview of the entire neck region.

TI-RADS (Thyroid Imaging Reporting and Data System) is a standardised scoring system used to classify thyroid nodules based on their ultrasound appearance. It assigns a risk category from TR1 (benign) to TR5 (high suspicion of malignancy). If any nodules are identified during your scan, they will be classified using TI-RADS, and the report will include a recommendation on whether further investigation (such as a biopsy) is warranted.
Ultrasound cannot definitively diagnose thyroid cancer, but it is the most important first step in evaluating suspicious nodules. It can identify features that raise or lower the suspicion of malignancy. If a nodule is classified as high-risk (TI-RADS 4 or 5), the report will recommend a fine-needle aspiration (FNA) biopsy, which is the definitive test for thyroid cancer. The vast majority of thyroid nodules are benign.
No, a thyroid and neck ultrasound is entirely painless. You will lie on the couch with your neck gently extended, and warm gel will be applied to the skin. The transducer is moved gently across the surface of the neck. Most patients find the procedure very comfortable and straightforward.
Your consultant sonographer will explain the findings to you immediately after the scan, so you will leave with a clear understanding of the results. A comprehensive written report and digital images will be provided on the same day, ready to share with your GP, endocrinologist, or specialist.
